SCHEDULING AN APPOINTMENT

If you are interested in beginning counseling, please contact me via email or phone to schedule our initial appointment.

Upon scheduling your session, you will receive a link to a client portal where you will enter your information. Forms include demographic information, insurance, and a general questionnaire. Completing the requested information prior to the session will help me understand how I can help you.

INSURANCE

I am in network with Blue Cross Blue Shield PPO and Blue Choice.

OUT OF NETWORK BENEFITS

If I am out-of-network for your insurance I can provide you with a monthly superbill/receipt upon request. Please contact your insurance company if you have any questions regarding your insurance company's out-of-network benefits. Any reimbursement from your out-of-network insurance company will be sent to you. 

Payment for all out-of-network clients is due at the time of service.  

PAYMENT

I accept cash, check and all major credit cards as forms of payment. 

CANCELLATION POLICY

If you are unable to attend a session, please contact me as soon as possible.  You may be charged a late cancellation fee for sessions cancelled less than 24 hours from scheduled counseling session time.
